We’ve recently implemented changes to our care plans for children and young people in order to make them more person-centred and easier to read.

The below is an example of our new care plan for a child and young person. They have been able to include their favourite signs for communication and images that they would like to include. Images make the care plan more engaging to read. The new care plan has also been reduced by nearly half the size, in order to make it more manageable to read.
