Active Care Group‘s core values are a commitment to putting the individual at the heart of everything we do, providing a seamless, personalised package of care that reflects the users wishes, hopes and expectations, taking the time to get it right.

Our boutique led approach is based on a foundation of detailed planning and strategy with all stakeholders to derive a person-centred care model shifting from limitations to assets, through this collaborative alignment we are able to maximise the positive outcomes for our residents.

Based in Purely, Surrey, Liberty Court can provide care and support for up to 24 adults between the ages of 18-65 who have epilepsy, learning and physical disabilities, mental health diagnosis and other complex needs. Our highly experienced staff provide 24-hour specialist support, maximising the standards, and positive outcomes on clinically led pathways of care for all our residents.

We are passionate about the promotion of high quality person-centred care delivery, assessing the most positive outcomes for our residents whilst working collaboratively with them, their families, and key stakeholders to offer bespoke clinically assessed packages of care. The care we provide is aligned to service user-specific, multi-award-winning in-house training and clinical nurse led governance. This ensures that each member of our specialist care team has the right mix of skills, expertise, cultural awareness, confidence, and passion to ensure the best care delivery, encouraging our residents to strive for independence.
