Birmingham
Specialist post-acute neurological rehabilitation allowing people to live rewarding lives.
Hunters Moor provides support for 33 patients at any one time with complex neurological or physical needs, who may require rehabilitation or reassessment of changing needs and long term, respite or end of life care. We support those with high dependency complex care needs, including but not limited to, respiratory, tracheostomy, stoma and catheter care, diabetes, wound and medication management.
Individuals are usually admitted directly from an acute hospital setting or Level 1 rehab setting once they are medically stable. At Hunters Moor, they are able to continue the rehabilitation they were unable to complete whilst in hospital.
Through our person-centred and evidence-based approach to rehabilitation, we aim to maximise independence and recovery whilst equipping people with the skills to return home or to a supported environment.
Individuals and their family work with the multi-disciplinary team (MDT) to set goals around their physical progress, functional abilities, future independence, leisure and work activities and communication.
We’re committed to raising standards and improving outcomes for our patients and have put in place a range of measures to establish a robust and integrated quality and governance strategy.
Patients benefit from access to our on-site doctor and neuro-specialist team consisting of speech and language therapists, occupational therapists, psychiatrists, psychologists, physiotherapists, therapy technicians, specialist nurses and rehabilitation assistants.
Specialist nurses are able to provide care to those with ongoing neurological nursing needs including respiratory and tracheostomy care, diabetes, epilepsy, wound management, stoma care and enteral feeding.
Our wonderful team understand that a holistic approach is necessary to ensure those in our care are safe, happy, and healthy. We promote and fulfil the company values of being caring, honest, pro-active, and empowering for those we support.
