Specialist Therapy provision across the Active Care Pathway
When faced with a change in health and wellbeing, it is essential that people receive the right rehabilitation at the right time. Therapies are offered across the whole of the Active Care Group Pathway and include a specialised team of:
- Occupational Therapists
- Speech and Language Therapists
- Clinical Psychologists
Our therapists work alongside clients in the community and those is our residential rehabilitation and care centres. They help identify a client’s potential to change at any stage of their journey and then deliver flexible and integrated therapy, building bespoke and sustainable rehabilitation around each individual according to their needs.
Our Therapy Model
Our therapy model is simple; we offer a team of skilled, experienced therapists, who apply their skills, measure the results achieved, monitor and adjust accordingly to create sustainable change and reduce therapy input over time.
Although the model is simple, it is also unique. Research has shown that the most effective and efficient results are achieved when teams work together and we truly believe that the whole is bigger than the sum of its parts, where traditional boundaries no longer exist and the entire team works in synergy.
This means therapy skills are shared across team members throughout the entire pathway. Whilst highly specialist skills are brought by individual professions, implicit collaboration occurs, resulting in shared beliefs and exchange of knowledge towards a common goal.
This bespoke ‘wrap-around’ system ensures the whole team works together to achieve our vision of sector leading, life changing care.
