Complex supported living care services.

Active Care Group’s complex supported living services offer person-centred care for adults with complex needs, including learning disabilities or autism. Individuals may have secondary mental health needs or long-term physical conditions.

The self-contained flats across our services encourage the individuals we support to live independently, whilst benefiting from care and support from experienced teams.

Personalised care

We provide compassionate and individualised care through Positive Behaviour Support (PBS). Our approach is centred on understanding and nurturing each individual’s unique needs to enhance their quality of life.

Specialist teams

The team, consisting of a Specialist PBS (Positive Behaviour Support) trained and experienced Operations Manager, Service Manager and Support Staff, provide 24-hour care, tailored to meet the needs of the individuals we support.

Inclusive planning

We ensure families and other key stakeholders, in addition to the individuals being cared for are involved in developing their own care plans and in choosing the way they live their lives whilst being encouraged to continue their hobbies, to try out new experiences and to learn new skills.

What is Positive Behaviour Support (PBS)?

Evidence-based approach

Positive Behaviour Support (PBS) is a proven, evidence-based approach focused on enhancing the quality of life of an individual with learning disabilities and/or autism.

Compassionate, personalised care

PBS uses comprehensive assessment and data analysis to identify the function of behaviours of concern, enabling the development of function-based, constructive strategies that proactively prevent these behaviours and effectively manage them when they occur.

Specialist support team

The PBS team at Active Care Group, including our Group Positive Behaviour Support Lead and Regional PBS Practitioners, work to deliver strategies within our services that will improve the quality of life of the individuals we support.

Our Positive Behaviour Support approach

Our staff teams collaborate with individuals, families, carers, and professionals to design and implement customised strategies. This ensures a consistent and supportive environment across all settings, from home to the community.

We focus on teaching new skills and replacement behaviours that serve the same function as the behaviours of concern ensuring individuals’ needs are met in safer, more appropriate ways.

PBS is a continuous process. We regularly monitor progress and make necessary adjustments to ensure care plans remain effective and responsive to individual’s evolving needs.
